Bipolar Hemiarthroplasty for Unstable Intertrochanteric Femur Fracture in Patients over the Age of 80 Years: Comparative Analysis between Cementless and Cemented Stem

چکیده
With today’s aging population, musculoskeletal diseases such as osteoporosis, degenerative arthritis, and degenerative spine disease, are on the rise, and these conditions are associated with increased fracture risk. Intertrochanteric fractures in elderly patients often involve severe displacement or comminution with poor bone quality. Accordingly, various treatment methods for hip fracture have been introduced. The incidence of fixation failure is reported to be as high as 20% in unstable intertrochanteric fracture due to nonunion, metal failure, and femoral perforation. Also postoperative ambulation may be delayed in this population due to the difficulty of achieving firm fixation. In addition, when fixation failure happens, reoperation may not be possible due to the patient’s general condition and medical complications. In comparison, bipolar hemiarthroplasty is performed as an alternative to osteosynthesis in elderly patients with unstable fracture patterns regardless of bone quality, and early ambulation is possible if stable fixation is attained. In cases of poor bone quality, cemented fixation was used in the past to enhance the initial fixation strength. A cemented stem is advantageous for stable fixation, early weight bearing, and less thigh pain, but it increases the operation time and blood loss, and complications such as cardiovascular toxicity. In the past, cementless fixation was not favored because it led to problems with initial fixation strength, bone ingrowth, subsidence of the stem and intraoperative periprosthetic fracture. Fixation of Trochanteric Fragments in Cementless Bipolar Hemiarthroplasty of Unstable Intertrochanteric Fracture: Cerclage Wiring
Purpose Bipolar hemiarthroplasty (HA) is an option for the treatment of unstable intertrochanteric fracture in elderly patients. There is a raising concern regarding cable-grip related complications for the fixation of trochanteric fragments. Therefore, the aim of this study was to evaluate outcome of cementless HA with fixation for the trochanteric fragments using monofilament wires in unstabl...

BACKGROUND Bipolar hemiarthroplasty for unstable intertrochanteric fractures in elderly patients is a viable option that can prevent the complications of an open reduction, such as nonunion and metal failure. This study evaluated the clinicoradiological results of cementless bipolar hemiarthroplasty for unstable intertrochanteric fractures in elderly patients. METHODS Forty hips were followed...
متن کاملCemented calcar replacement versus cementless hemiarthroplasty for unstable intertrochanteric femur fractures in the elderly.
BACKGROUND Unstable intertrochanteric fractures remain a challenging problem in elderly individuals due to high failure rates associated with internal fixation. Hemiarthroplasty is one treatment option for intertrochanteric femur fractures in elderly patients. The aim of the present study was to compare the reliability of cementless and cemented hemiarthroplasty for unstable intertrochanteric f...

PURPOSE To compare and analyze clinical and radiologic outcomes of cemented versus cementless bipolar hemiarthroplasty for treatment of femur neck fractures. MATERIALS AND METHODS A total of 180 patients aged 65 years and over older who underwent bipolar hemiarthroplasty for treatment of displaced femur neck fractures (Garden stage III, IV) from March 2009 to February 2014 were included in th...
متن کاملThe effectiveness of primary bipolar arthroplasty in treatment of unstable intertrochanteric fractures in elderly patients
BACKGROUND The treatment of unstable intertrochanteric fracture in the elderly patient is still controversial. Traditionally, internal fixation using a dynamic hip screw was of choice. Recently, some authors advocated the use of cemented bipolar arthroplasty or hemiarthroplasty which results in better functional outcome. AIM The aim of this study is to find out which of these treatment option...
